Your Role

As a Registered Nurse (RN), you will deliver skilled nursing care to patients in their homes, following individualized care plans and working closely with physicians, families, and interdisciplinary teams. This position is ideal for a nurse who thrives in a flexible, independent work environment and is passionate about patient-centered, one-on-one care. You’ll play a key role in improving quality of life, building strong relationships with clients and their families, and ensuring that each person feels respected and cared for. This role may involve working in home care, assisted living, or healthcare settings depending on client needs.

 

What You’ll Be Doing

• Conduct in-home nursing assessments and develop individualized care plans

• Provide skilled nursing care, including wound care, injections, medication management, and chronic disease monitoring

• Monitor and document patients’ progress and communicate updates to physicians and care teams

• Educate patients and caregivers on disease management, medications, and post-hospital care

• Ensure compliance with all clinical protocols and state/federal regulations

• Respond promptly to patient needs and emergencies

• Collaborate with agency staff, care coordinators, and other healthcare professionals to support the patient’s overall health and well-being

• Pay rate based on experience, availability, and clinical background

What We're Looking For

• Current, active RN license in the state of California

• Minimum of 1 year of clinical nursing experience (home health experience preferred)

• Current CPR certification

• Reliable transportation and valid driver’s license

• Strong clinical judgment and documentation skills

• Ability to work independently and manage a flexible schedule

• Excellent communication and interpersonal skills

• EMR experience a plus (training provided if needed)
   

Physical Requirements

• Ability to lift up to 50 lbs and perform physical tasks such as bending, standing, and transferring clients

• Capable of getting down on the floor, kneeling, crouching, or quickly responding to clients as needed
